 1.   Name of the Organization requesting for ambulance: 


  Sher Bahadur Shrestha, Pratisdhan, Nepal, Urlabari-1, Morang is a non-profit, non-governmental as well as service-oriented organization which is registered in District Administration Office, Morang on 20 August 2018, District Co-ordination Committee and Social Welfare Council in Kathmandu.

 2.   Details of the organization:

       It was established on 20 August, 2018 with the joint efforts of local volunteers, social workers, teachers, journalists, farmers and other devoted youth groups.

       Founding members of this organization is 9  members.

    Currently, this organization has formed different committees for uplifting the living standard of the local people.

Objectives

     The main objective of this project is to serve the needy people of remote areas of Nepal with fast and easy health facilities and reduce maternal mortality and morbidity rate.

    The specific objective of this project is

     1.  To use one 4-WD ambulance to carry the sick persons of hilly region and Terai Region.

     2.  To provide technical support to the staff of proposed medical centers for fast and emergency treatment of patients.

     3.  To provide necessary basic medical equipment and first aid materials to health centers of project sites.

     4.  To maintain environmental balance in the surroundings.

     5.  This organization will operate ambulance service and Corpse carrying service with the approval of the concerned body keeping this area in mind.

     6.  To conduct public awareness programs related to health services at the local level by adopting human services.

    The total number of Members of this organization is 500 people. They are active, hardworking, dedicated in their duties and responsibilities.

    Funding Mechanism

    This organization has a distinct fund in which the following includes:

     a. Subscription fees and renewable fees of the membership.

     b. Optional donation and legacy.

     c. Subsidy and grants provided by Nepal Government

     d. Donation, Subsidy, and grants received from local governance body.

     e. Grants, Donation provided by Donor agencies or another person

    Activities

a.   Relief materials were distributed to the marginalized people.

b.   Raised awareness about COVID-19 to the people.

c.    Performed the activities for preserving forest areas.

3. Region (Province/District/Municipalities/Ward/area) in which the Organization is operating:

       Morang is one of the 77 districts of Nepal and falls under Province no. 1. Morang district covers an area of 1,855 km2 and has a population of 965,370. (2011 AD). The provincial capital of Province no. 1 is Biratnagar which lies in this district.

       Urlabari Municipality, located 55 km northeast of Biratnagar, the district headquarters of Morang district, was declared as a municipality as per the decision of the Government of Nepal dated 2071-01-25. Madhumala VDC Wards 7, 8, and 9 have been integrated and implemented as Urlabari municipality. It is bounded by Damak Municipality of Jhapa District in the east, Patharishanishchare Municipality and Letang Municipality in the west, Miklajung Municipality in the north, and Ratuwamai Municipality in the south. It covers an area of ​​74.62 sq km. Has been It is located at 26.67 degrees north latitude and 87.61 degrees east longitude above sea level. Out of a population of 70,605, the town has 13,761 households including 51.13 percent women and 48.87 percent men. Although most of the land is arable land, increasing urbanization has necessitated a systematic plan for the development of the region. The old forest area is known as Urla (tiger) and Wadi (place) or tiger habitat, two words of the Santhal language can be considered to have developed as an urban area with the gradual increase of human settlement. The road from Urlabari Chowk to Ravi of Panchthar via Madhumalla in the north and an alternative road directly to the district headquarters in the south also shows the potential for its development. Due to the existence of Betini wetland, Budha Thakur, Lampate wetland and the efforts made at the municipal and local level in its promotion, Urlabari municipality seems to have more potential in tourism development.

 

4.    Whether the area is rural or urban.

       The area in which the organization is operating is urban. It lies at Mahendra Highway. The area includes Upper Terai, Inner Terai and chure hill.

5.  Condition of road/connectivity in service area/s and hospital/s nearby:

       Urlabari Municipality has blacktopped roads to its major areas. But other parts of this municipality have not got a good condition of the road to the village side. The road to the hospital is in good condition to reach.

        Madan Bhandari trauma centre has 100 beds. Godawari hospital has 50 beds. Araniko Pvt. Ltd has 20 beds.

None of the hospitals are providing ambulance services. All the hospital have lack of ambulance. Currently, there is no vehicle facility to transfer the patients to the district hospital. Due to this, people are forced to pay huge sum for alternative transportation or risk of reaching hospital late. The lack of proper transportation from these rural municipalities to the district hospital is far severe in case of women in labor. Numerous cases of women deaths during pregnancy are recorded in its surrounding rural municipalities.

7.  Other organisation/charitable institutions providing ambulance services in the area of operation of Institution/Organization:

................... is providing ambulance service to this sector. The ambulance managed by Co-operative is old. It has completed 14 years. In the place of this ambulance, next ambulance is required.

 

                ii.        Other details:

The ambulance is not fulfilling the demand of the people in this area. The number of patients or sick persons are increasing rapidly. Its service is not sufficient for the people of this area and its surrounding rural municipalities. With the lack of sufficient ambulance service, the patients from this municipality and rural municipalities – children, women and old alike, are unable to reach the hospital on time and increase untimely death.

The ambulance operating committee will prepare all the guidelines for using an ambulance. It decides different types of expenses and fees incurred on behalf of Ambulance.  

 

                ii.        Source of funding for Operating and Maintaining the ambulance:

The ambulance operating committee will collect money as 10% out of the income earned by ambulance for maintenance, repair, and replacement cost.

 

              iii.        Documentary proof in support of the financial position of the Organization:

The financial position of the organization is sound. All the fees collected from members will be deposited in its fund. It has opened Renewable funds. This foundation has opened a Bank account in .................... The account of this foundation is ............members amounting to Rs. ...............                   ii.        Relevant details:

People living in hilly areas are not able to get health care services. Because of the delay in treatment, a compelling situation has been created where one has to die untimely. Government and non-government private hospitals and clinics established in Letang Municipality are operating in deteriorated conditions. Due to the difficult topography, it is difficult to get timely medical treatment for the people of the community.

 Even in the big cities and district health centers, there is a lack of modern treatment facilities. In a few health centers due to a lack of trained human resources, medical equipment remains unused. So it is necessary to provide training to the health personnel on modern equipment and its handling procedure so that peoples in that region will get easy access to the modern health facilities at a cheaper rate in a short duration of time.

 The municipality has a dense population of Indian Army and present working Indian Army’s families. This area supports the Indian Army's families. The ambulance service also gives the services of Indian Army Families and the Indian Army.

 

13.  Details of the Committee with your Organization to oversee proper Operations and Maintenance of ambulance:

Ambulances will be handed over to the ambulance operating committee of Sher Bahadur Shrestha, Pratisdhan, Nepal under the supervision of the respective Executive committee of the organization. The monitoring of the ambulances provided by the Embassy will be inspected by the respective Committees and members of the organization. The ambulance operating committee will have a great role in this Ambulance project. They have roles from the beginning of the project, from the identification of the need to the implementation of the project and also after the completion of the project for its sustainable operation. The organization will form an Ambulance Service Management Committee and will operate the ambulances under this committee. They will also be responsible for the management and operation of the ambulances. Since the ambulance will be used for the quick response of the patients and the organization will be responsible for the maintenance and sustainable utilization of the ambulances.

Regarding the fees for transporting the patients includes minimum charges covering driver’s salary, administration expenses and maintenance and repair expenses. Comparing the charges of other ambulance, a minimum charge will be taken.

The report of condition of ambulance will be submitted to Executive committee of the organization and to the embassy of India weekly, fortnightly or monthly after monitoring.

14.  Areas to which the ambulance services will be provided:

 

                    i.        Rural Municipalities:

Kerabari rural municipality, Miklajung rural municipality, Madhumalla rural municipality will be the direct beneficiaries of this project. These rural municipalities have no service of the ambulance.

The total population of this municipality and other rural municipalities is 5 lakh approximately.

          Urlabari is a town and municipality in Morang District in the Koshi zone of south-Eastern Nepal. According to New provision of administative division, this city lies in Province No. 1. It is considered to be the sencond largest city in Morang District after Biratnagar, the capital city of province No.1. At the time of the 1991 census, it had a population of 18,224. At the time of the 2011 census, it had a population of 35,166 and 8,165 households.

         It is located 60 km eastern part of the metropolitan city Biratnagar. According to the declaration as a municipality from the government on 2071-01-25 to Urlabari, Morang. Jhapa lies in the east side of this municipality, oriented to be known as a beautiful city of Morang district. From 2071 census, the total population is 54,696 where 29,548 are the women and 25,148 are men lived with 12,530 households.

         Urlabari is surrounded by Letang Municipality in the west, Damak municipality in the east, Miklajung Rural municipality in the north and Ratuwamai municipality in the south.

         People living in hilly areas are not able to get health care service. Due to not getting treatment in time, a compelling situation has been created where one has to die untimely. The service provided by Government and non-government private hospitals and clinics established in northern side of Municipality are not satisfactory. Due to the difficult topography, it is difficult to get timely medical treatment for the people of this community.

 

Although there are government bodies and non-governmental organizations in Urlabari Municipality, they have not done much to raise public awareness about medical treatment and health issue among the target groups and communities in rural and urban areas. Most of the organizations are unable to focus on this issue seriously. The energetic, meticulous, devoted, enthusiastic members and founder members of the SherBahadur Shrestha, Pratisdhan have taken this issue seriously and immediately raised public awareness about health which is demanded by the people of different areas for providing health services accessible to all communities.

The majority of the population living in and around this area is densely populated by former Indian Army and current serving Army families. Statistics show that almost 30% of the total population here is from Indian Army families. People from here and other areas have been forced to enlist in the army due to a lack of employment opportunities.

Moreover, Urlabari Municipality lies in Morang District. Morang District has easy access to get to India for joining the army force because its border is connected with the Bihar State of India.
